Location: Los Angeles, CA
Position: LVN/LPN Palliative Care 
Remote/Virtual Position : No  
Coverage Area: Southeast Los Angeles County, including Downey, Pico Rivera, Montebello, Commerce, East Los Angeles, Maywood 

Salary: $23.00 - $36.00 per hour
Schedule: Full Time, M-F

Responsibilities

If you meet these qualifications, we want to meet you!

• Graduated from an approved school of professional nursing
• One year experience as a licensed practical nurse / Palliative experience preferred, required in California

• Licensed to practice as a Licensed Practical Nurse in the state of agency operation
• Possess and maintain valid CPR certification while employed in a clinical role, preferred
• Must be a licensed driver who can travel to all business locations 
• Meet the regulations and requirements of the state(s) in which program provides services

Responsibilities: 

As a Palliative Care LPN / LVN, you will: 

• Under the direction of an NP or physician, assist in execution of the care plan, including routine and emergent visits

• Contributes to the on-going care planning process, especially regarding medical needs, with the care team and other agencies as required
• Initiates and maintains communication with the NP, interdisciplinary team, Palliative Director, and contracted medical groups to coordinate optimal care and use of resources for the patient/family
• Proves support for the Palliative patient and family as new clinical needs are identified or questions arise, with the referral to the clinician as appropriate
• Gathers information regarding the patient for appropriateness for other service lines that will best meet patient needs such as Hospice
